About the Service

Our Hospital Discharge Support service links people to community, voluntary and private sector services that can enhance their independence and day to day lives on their return home from hospital.

Our Hospital Discharge Support service provides a range of services to help people after discharge from hospital. We concentrate on practical help that maintains independence and wellbeing. We work alongside University Hospitals Sussex discharge teams to do this.

Our resources include:

  • Grants to pay for works to make homes suitable to return to
  • Community support and help to access community services to ease the transition from hospital to home
  • Wellbeing phone calls, to check on people who have left hospital, referring them to local services or assistance contacting the services that they need.

We combine access to hospital discharge funding so vulnerable patients can be safely discharged from hospital with ongoing support from a link worker. This helps people to become more independent and recover at home.

Providing support in this way reduces the likelihood of readmission to hospital.

The service is funded by NHS Sussex Integrated Care Board (ICB) and Brighton and Hove City Council’s Disabled Facilities Grant.

Possability People both support with immediate practical tasks to get individuals home – and follow up with support to help find services or provide advice to promote patients to stay at home as independently as they can.
